Benefit for injuries caused by a covered accident is subject to a 7 day elimination period. Up to $2,500 or $5,000 maximum per year. Paid at $500 per day. No elimination period for sickness.

• Critical Illness Provides a lump sum benefit amount upon diagnosis of a variety of covered critical illnesses, including: Invasive Cancer; Heart Attack; Stroke; Coronary Artery Bypass Graft; Major Organ Transplant; Kidney (Renal) Failure; Severe Burns; Paralysis; Loss Of Sight, Speech Or Hearing and Coma. Up to $2,500 or $5,000 per occurrence. Lump sum payment. Can provide coverage for Additional Occurrence and Recurrence. Pre-existing condition limitation applies.
• Accidental Death or Dismemberment Provides a lump sum benefit amount for death that is the result of an accidental injury which occurs within 365 days of a covered accident. Scheduled benefit amounts for dismemberment are also included. Up to $2,500 or $5,000 maximum death benefit. Dismemberment benefit per schedule.
